Community Paramedicine brings clinical assessment, treatment, monitoring, diagnostics, and care coordination into the home and community setting.
Current Partnerships & Collaborative Initiatives
Community Paramedicine works alongside healthcare providers, community organizations, and health system partners to develop practical solutions that improve access to care and bring services closer to home.
Current initiatives include:
- Hastings Prince Edward Ontario Health Team (HPE OHT): Collaborative initiatives focused on improving access to care across the region, including primary care integration, referral pathway development, palliative care planning, and regional clinical initiatives such as the spirometry pilot
- Canadian Mental Health Association (CMHA): Integrated outreach services supporting individuals experiencing homelessness, substance use challenges, and complex health and social needs through the Homelessness and Addiction Recovery Treatment (HART) Hub.
- Belleville and Quinte West Community Health Centres: Collaborative community-based clinics, health promotion initiatives, and preventative care programs that improve access to healthcare services.
- Hastings County Housing Services: Community clinics and health promotion initiatives delivered directly within housing sites to support resident health and wellbeing.
- Retirement Homes: Collaborative healthcare services that improve access to clinical assessment, monitoring, treatment, and follow-up care for residents.
- Primary Care and Specialist Partners: Collaboration with family physicians, nurse practitioners, specialists, and healthcare teams to bring clinical assessment, diagnostics, monitoring, treatment, and follow-up care closer to home.
